A. FULLY VACCINATED PATIENT / COMPANION
-Present a valid ID and a copy of your vaccination card or Vaccination Certificate (https://vaxcert.doh.gov.ph) upon arrival at the clinic reception desk
-Wear a face mask and face shield. Removal of face shield only for medical procedure and as guided by our personnel. Face masks and face shields are required in a medical facility setting (IATF Reso 149, 11/18/21)
B. UNVACCINATED/PARTIALLY VACCINATED PATIENT / COMPANION
-Present a negative antigen or RT-PCR test (results within 48 hours of visit); OR
-Undergo antigen testing at the Asian Eye clinic and receive the results within 15 mins. Antigen testing is Phpxxx
-Advise the Customer Service Representative you spoke with if your eye concern is urgent

ADDITIONAL FAQS:
1. Why are unvaccinated or partially vaccinated patients or companions required to submit a negative antigen / RT-PCR test?
A clinic is an indoor setting where a higher level of protection is important because you may be near other patients who are older, or with medical conditions that put them at higher risk of severe COVID illness.
2. What if I test positive on the antigen test administered at the clinic?
a. When you test positive on an antigen test, take an RT-PCR test to confirm.
b. Isolate, even while you are waiting for results of an RT-PCR test.

c. Contact your primary care provider for assistance during the required 10-day isolation for positive cases, regardless if asymptomatic, mild or moderate.
3. Are there rules on what kind of mask I can use?
An effective mask has both good fit and good filtration because it improves protection for others and for yourself.
A well fitted mask has no gaps between the face and mask, at the side or above the nose. Gaps put you at risk of virus particles leak in or around the sides of a mask.
